Why it caught on: the picture shows a young woman grinning widely, with blood smeared over her mouth, chin, neck and shirt, standing in a car park. People used it for a two-line format: a sweet or ordinary setup on top, a grisly punchline underneath. The joke was the old "psycho girlfriend" stereotype, the jealous, obsessive partner who turns violent. It spread because the cheerful face made even the darkest punchline sound like good news.
