Your friend sets up a perfectly innocent sentence with a long, confident pause. You know where it's going and you let it go there anyway. Nobody is ever ready for the second line, and that is the whole appeal.

Why it caught on: the picture is a glossy, smiling portrait of a man with a tight curly perm and a sparkly blue vest on a golden backdrop, and its bright, wholesome look made it ideal for a two-line setup with a crude punchline. The caption template did the rest, and anyone could write a sentence that turned halfway through. The stereotype behind it was the caricature of a gay man as flamboyant and permanently thinking about sex, which is what most of the original captions leaned on.
